Diablo 4 players are losing Wirt's Lucky Coins to a Season 15 bug
The coin can disappear at the Old Tristram fountain without opening the Echo of Tristram portal.
If you are currently hunting Season 15 
Hellfire Torch in Diablo 4, chances are that you might run into a particularly painful bug that can eat Wirt's Lucky Coin without opening the portal to the boss fight.
The problem has now been reported by multiple people, with the coin disappearing after being used at the fountain in Old Tristram, and the expected portal to Echo of Tristram never appears. That means the three hard-earned Prime Evil Splinters used to create the coin are effectively lost as well.
A Fountain and a Missing Coin
Diablo 4 Season 15 has a full-on secret quest that leads you to Tristram. We will detail that in another news, but basically, what you need to know is that there are 10 Deckard Cain journals to collect in order to get a portal that leads to Tristram. After that, you also need to collect three special items:
Splinter of Hatred
Splinter of Terror
Splinter of Destruction
Putting all three into the Horadric Cube creates Wirt's Lucky Coin, and these can then be used at the fountain in Old Tristram to open the portal to the secret boss encounter.
The problem is that the fountain does not always seem to complete the second part of the process.
In some cases, when you use your Lucky Coin at the fountain, you see it disappear from your inventory, but you get no portal at all. Sometimes you can even end up not being able to interact with the Fountain at all anymore.
This is definitely not an isolated incident, because it has now been reported by a lot of players, and there is sadly nothing else you can do except farm for Splinters again, get a new Coin, and try one more time.
Hours of Farming Gone
The particularly frustrating part is that Wirt's Lucky Coin is not something you can simply buy or craft from ordinary materials. You have to farm the three different Prime Evil Splinters every time before you can make another one.
That means a failed fountain interaction can effectively erase the progress needed to access Echo of Tristram in the first place.
There is currently no confirmed explanation for why the portal sometimes fails to appear. The issue could be related to the shared Old Tristram instance, particularly when multiple people are attempting to use the fountain around the same time. That is still only a community theory, however, and Blizzard has not confirmed a cause.
There are also reports of the issue happening before and after the recent Season 15 hotfixes, so it is not currently clear exactly what is triggering the failure.
For now, the safest option is to hold onto your Wirt's Lucky Coin if you do not want to risk using it. Tristram is not going anywhere, so you have the entire season to still do it at least once.
The Hellfire Torch Is Worth the Trouble
The reason everyone is going through all of this in the first place is the reward waiting inside Echo of Tristram. The secret encounter brings Diablo, Baal, and Mephisto together in a single fight, with the Hellfire Torch among the rewards. So it is really worth doing, but maybe just wait to see if Blizzard releases a fix for this or not.
Unfortunately, right now the hardest part might not be surviving the Prime Evils. It might just be getting the damn portal to open.
