Diablo 5 will tell three stories at once, and Sanctuary will keep changing
A region could be reclaimed by humanity or taken by another Dread Commander.

We already knew Diablo 5 would throw us into a Sanctuary where Diablo has won, humanity has mostly fallen, and the world itself can change between visits. This information was shared with fans during BlizzCon 2026, so that in itself is not new. However, since then, Blizzard has explained a little more about what all of that is supposed to mean.
Speaking to TechRadar Gaming after the Diablo 5 reveal, Senior Game Director Joe Shely said the team is telling "three stories" through this version of Sanctuary: its past, its devastated present, and a future that is still being transformed by the forces of Hell.
And that last part could make returning to places we have already explored much more interesting.
We Are Going to Learn How Diablo Won
Diablo 5 begins after the worst has already happened. Diablo has conquered Sanctuary, Westmarch has burned, and there are only a few humans left who have not been killed, corrupted, or forced into hiding. Blizzard's official overview describes the Western Continent as the heart of Diablo's reign, with the Lord of Terror actively reshaping Sanctuary from his throne in Westmarch.
But the game is not simply skipping over the fall itself.
Shely explained that the past will show us more of how Diablo and his Dread Commanders took control in the first place. In another part of the same TechRadar interview, he confirmed that we (the players) will learn how everything came to be as it is as we travel through the game, rather than Diablo's victory happening entirely off-screen.
The present is the Sanctuary we begin playing in. It is almost empty, devastated, and the remaining survivors become part of the caravan we bring with us.
Then there is the future.

Dread Commanders Can Completely Change a Region
Blizzard calls this third part "terror forming."
Dread Commanders can transform the regions they control into their own version of Hell, meaning an area does not necessarily stay the way we first found it.
Narrative Director Jennifer Hepler gave one particularly interesting example. A location controlled by the Dread Commander of Undeath could become overrun with zombies. Return later, and humanity might have reclaimed it. Or another Dread Commander may have taken control instead, changing the environment again.
So this is not simply about Diablo 5 shuffling some monsters around every time we return.
Who controls a place can potentially change what that part of Sanctuary looks and feels like.
Sanctuary Itself Is Telling All Three Stories
This also explains Blizzard's earlier description of Diablo 5's procedural world a lot better.
The official BlizzCon recap already confirmed that terrain, points of interest, monsters, and rewards can change when we revisit an area. And what is even more important is that Blizzard described its ruins as telling the story of what was lost, its corruption showing Diablo's current rule, and the forces of Hell giving us a glimpse of what comes next.
Now we know those are not just three bits of environmental flavor, so to speak. It all works together in a way that makes sense with the three parts of Sanctuary's story that Shely described.
Diablo 5 is still very early in development and is currently planned for Spring 2029, so there is still a lot of time for these systems to change, but the idea of what Blizzard wants the game to be is already quite clear; We are not just exploring the aftermath of Diablo's victory. We will be discovering how it happened, surviving what it created, and potentially watching Sanctuary continue to change around us.
